Not being in first means means you get a fairly large speed boost from slipstream and you will get items that allow you to take shortcuts regularly. That combined with items specifically meant to hurt the person in 1st makes it really hard to build up much of a lead unless you get lucky with people behind you pulling each other down with item hits.
