From my understanding, NPCs are far more dark and nefarious than the simple video game equivalent. They are 'activated' from birth and are in the hive mind of the demiurge. They can see the 4D and 3D simultaneously, and seek out souled ones as their food. Ones with souls are blind to this and only know of the 3D since their memories were wiped, not knowing this place is illusionary; this place runs on their soul energy.
