there are tons of publicly readable buckets out there that are accessed by backend services, but these are somewhat obscured because you can't just read the frontend document to know where they are. the fact that someone actually found this bucket suggests that frontend scripting was pulling these files directly from the bucket-- which is not just insecure, or negligent, it's basically offering up the user data and begging people to take it.

Buckets are often not even purely a backend service, the frontend interacts with them. It's pretty common to upload/download directly from the frontend using presigned URLs, probably what happened in this case.
It would be the type of thing you would easily see just while scouting a site to see what servers it's hitting.
To answer your question, hackers interact with backend services pretty commonly to look for insecure misconfigurations exactly like this.

All you have to do is attach any cloud service to a public IP to see illegitimate access requests start to come in. Cloud companies have only certain ranges of IPs reserved and these ranges are widely known. Yes you can scan all of them pretty easily.
That's one example of how you can get access to a service you're not supposed to have. Not targeted and it has to be public (although assume even if you're using a VPC they'll try every key they have on your gateway) but I'm sure there's other, more targeted ways to get access to a service.

It probably means you're uploading/downloading files twice, once on your server and then once on the bucket.
If your backend is on the same cloud service as your bucket though it's probably pretty fast. I haven't done serious testing.
Depends on your use case I guess. On the one hand it might be more secure, provided you actually have policies to only provide access the bucket through your VPC(s). On the other hand you have to add your own file upload/download mechanism beyond just getting a presigned URL.

Apps aren't made by one person. You could EASILY have one clueless person set up file uploads early on. At that point all you need is no one on the team who's security conscious have to touch that part of the system, and not have them audit the system.
