The issue is the person can share the evidence of the attack.
More scary than attacks through AI generation is the idea of using it to cover up any transgression through keeping a simulacrum of you alive and fine.

You could be sent to a work camp while a model answers all your calls imitating your voice and mannerisms, all your socials are updated automatically.
Even close ones could be fooled with all the privy knowledge that has been mined from chats people thought were actually private.

Anyways if the idea is triggering some kind of mental breakdown in most neurotypical people, it doesn't necessarily have to be a spooky image or video.
Could be something that looks totally harmless or even nondescript to someone that isn't affected, as long as it triggers and imprints certain areas of the brain, maybe some scifi-tier advanced AI could be able to generate something like this.

A still image? 0% chance.
An animated one? 0.01% chance.
A targeted animated one based on a persons psychology or brain scans. 1% chance (if they are old).
A weird shifting pattern, that's not technically horrifying but hacks the human brain and makes it short circuit? 1% chance for the general population young/old.
A full blown auditory visual room or VR headset that can't be removed, and locking them in there for a full day? 10% chance.

I think its ability to create cognitohazards is going to be inherently limited to the garbage in, garbage out paradigm.
It will need training data and restrictions that let it parse a noise input into a cognitive hazard, which presupposes starting from a position where someone would be able to make a cognitive hazard without AI.
