often feel like I have a very tenuous grip on sanity, like I am just barely keeping it together

and when I say just barely I don't mean that I present as a normal person, I mean when people look at me they can see that I am visibly mentally unwell. Not in a 'twisted freaken psychopath' edgy way but in some way that consistently makes other people uncomfortable, as if I'm not someone to be spoken to or stood near, as if I'm "a bit off", agitated somehow.
