English 100%. I’ve seen it 11 times and only 1 of them was in Italian. Tony Franciosa’s performance is my favourite in Dario’s entire oeuvre and a lot of it is in his singular vocal cadence. John Saxon and John Steiner do their own voice work too. It’s a story about an American in Rome surrounded by his local handlers so it makes sense to be in English, and all the actors spoke English on set.
