I played it awhile back, it wasn't anything special.
Compared to Pisces, Johto was only mildly remixed with some additional areas or total area overhauls sprinkled in, and you still end up progressing through the region in a similar order to the actual Johto games (following the "you go left after Ectruteak" path that's become commonplace in the fandom rather than nonlinearity, of course) with mostly brand new gyms and the rare returning face, though rather than going to Kanto once more, you get a little original series of islands that aren't too special (especially on account of the fact that your access to Megas is locked to postgame, thus the postgame was tasked with hamfisting all of the Mega Stones into its dwelling, so one area is literally just a series of optional gauntlets for some of them because there were simply too many to account for).
Speaking of Megas, wouldn't know about how Meganium, Feraligatr and Johtonian Typhlosion function, but Mega H.Typhlosion was utterly hilarious because it gets Contrary, allowing you to brainlessly spam Overheat and Eruption in tandem to sweep virtually everything in postgame.
It's a dexbloat game too, so you don't really get a good feel for Johto being Johto because everyone's busting out a clusterfuck of mons from every generation up to 8+PLA.
Story isn't anything mindblowing, but your rival being your douchebag older brother does manage to remain pretty funny when it keeps landing him in deep shit.
