I honestly think the answer to this is that she lets him fuck whoever he wants to fuck. They must have some sort of agreement. In fact she probably participates and finds him young clean women who want to fuck Jeff (in exchange for money, etc. of course).

My best guess is that she's basically a female pimp for him and saves him the hassle of dealing with unknowns, i.e. random gossipy women, blackmail, ultimatums, monetary demands and all that. He feels comfortable with her at a personal level enough to charge her with satisfying his sexual and domestic desires, thus she's his wife. Probably a smart move on his part. It'd save a lot of headaches and hassle
