It's a scripted stunt performance. The flips and the shoves in the ring are definitely fake (and more than a little gay) but you also can't exactly fake falling 16ft straight onto your spine. There's not a lot of padding you can use in those outfits and most of the people who did the stupider "trash wrestling" stunts resulted in permanent, long-term injuries.
