They are either being paid by the free healthcare corporation which subsidizes its services partially or fully to its clients, or the clients are compelled to pay the full price to the private sector once they realize how shitty is the mandatory "free" healthcare.
It's kind of like when you're doing an annual vehicle inspection test for your car, and the place that does it is located in a complex with lots of other autoshops whether for flat tires, electric issues, etc.
They're all in it together.
