It's a Logitech G502 Hero "special edition." Got it on Amazon for like $20 five or so years ago.
Looking up the issue now, apparently they infamously break after like one year, but mine went the distance. But it's also something I can't fix without cracking all the glue open and soldering a switch. So I'd rather just spend $20 on a new mouse.
