It's real and Sam himself brings it up every now and then in his stuff. Those neck braces Sam used in his early bits weren't just props he bought off ebay. He was a reckless driver in his youth and gave his best friend at the time brain damage in a wreck.
And Sam in his weasily way tried to make good years later to absolve himself of the karmic debt he incurred for that but it didn't work because he was probably never genuine about it.
