It could be that a dark world changes based on its creator's will. Kris/Carol wanted a dark world where the dead come back to life, whereas Susie just wanted to find someone who knew the code (which ended up being the fire extinguisher, hence Gerson was unnecessary). The Knight doesn't appear in the second sanctuary from what I recall.

Susie THOUGHT Gerson knew the code but he actually didn't. He had been dead for a while, his son could have very well bought the lock since then.
We don't know who exactly placed the lock there - perhaps Carol stole one from Gerson's son's desk. Why she would bother to put a lock there in the first place is another mystery - I guess to separate the first and third sanctuaries? Why, I don't know.
