What about a hybrid method?
Render primitive, simple meshes in real time, and use essentially a glorified inpaint generative AI to generate the lighting and details. You could even (slowly) render a more detailed "reference" image and feed it into an AI to make sure things look right.

That's far more realistic of a use-case scenario. Using it as a fancy renderer with a properly grounded truth instead of trying to wholesale create things from pure noise.
However, developments in gaussian splat's are far more likely to overtake for "Muh realism", and if creative types were willing to go full miniature / stop motion, would be perfectly fine for more abstract looks as well.
