Most harder difficulties are made with the idea that the player is already familiar and experienced with the game to some degree. RE7’s hardest difficulty, remixes item locations, has the harder enemies show up earlier, etc.

majority of the games that do this have the locked difficulty balanced for NG+ where you have your entire endgame skillset from the start

it's because of game journalists
normal people have the sense to pick the difficulty they want, but if journalists saw the hardest difficulty they might pick it and give the game a low score. or the fact that the hardest difficulty exists would make them feel insulted for playing on easy, so they would give the game a low score
