My biggest dissapointment is that, at this rate, any new content will be exclusive to floor 5 and above so every other non-superposition run will be all the same and at some point, we won't even get new gifts down there either. Much less abnos or more enemy variety because, again, locked to Floor 5 hard MD and superposition.

Or maybe metaphors might be a good way to think of them as well.
To use some other examples, Shelter isn't literally some place made by P Corp, it's an abno that is the manifestation of people's perceptions about P Corp, materialized into reality from the River. Or to use a Limbus example, Wayward Passenger isn't literally a W Corp Agent that somehow got lost in the warp, but most likely the unconscious fears of W Corp agents about such a possibility happening, made material.

What's the issue with Red Hood? It's as you say, most likely born from the story of red hood.
We actually see the abno creation process in Lob Corp, although Binah uses rather flowery language since she's Binah. But the gist of it seems to go something like:
Carmen as the bucket and source produces all the cogito, the 'water' of humanity, making her the 'mother of all abnormalities' as Binah calls her.
Binah uses this cogito/'water', and injects it into humans at very precise levels.
If the process goes correctly, an abno will be created, the specific abno depending on the person/water.
