The whole thorn right scene being a possible rape metaphor reminds me of that scene in Mother 3 where Lucas learns PK Love in the hot springs with that Magypsy.
What literally happened was Lucas had to hold his breath/meditate underwater but the cut to black and suggestive dialogue makes it look like something sexual happened. Obviously nothing actually sexual happened or the game wouldn't have been rated All Ages in Japan, it was an adult joke for immature minds to have a chuckle at.
Deltarune is heavily inspired by Mother 3, so I wouldn't be shocked if Toby wanted to make a darker take on that scene.
Everything that literally happens is par for course but you can't shake the feeling that something way more fucked up happened because of the framing.
