Toby has had this game in his head for more than a decade
Everything wasn't fully drafted/outlined until after Chapter 1 but he's posted drafts and concept art of specific scenes going all the way back to 2012

Toby did not have a rough draft of the ENTIRE script of the game prepared in 2012. Not even close.
Toby had his fever dream showing him "the ending of a game" he became inspired to create in mid-2011. That was only the start of Deltarune's conceptualization, and he took inspiration from many other things he was working on and seeing at the time (Homestuck, Kanotyne's Card Kingdom designs) and in the years since (Undertale).
Toby kept thinking about Deltarune while working on Undertale but work didn't begin on it properly until some time after Undertale's release, and Chapter 1's development only laid the foundation for the rest of the game as Toby realized he was in over his head and if it was just him and Temmie and a few others trying to do this the game would never come out.
The project was basically rebooted (almost literally since they seriously considered switching from GameMaker) when the actual Detlarune development team was brought together, and Toby spent a lot of that time between 2018-2020 working out how the rest of the game would actually go. Even then, we know that some significant changes have happened - we know that Queen was originally going to be a more serious villain before she became more comedic, Spamton NEO was born out of plans for a more proper Mettaton NEO fight that didn't originally involve him, and while Tenna was probably always meant to be some kind of foil to Spamton some aspects of that definitely got played up in light of Spamton's runaway popularity; in particular the entire "Mike" character was seemingly invented in response to fans running away with the idea after Chapter 2.
TL;DR Deltarune has been in development in some form for almost 15 years, it has changed a lot over that time and is still not set in stone even now.
