It's like Phantom Pain. Revenge, loss and PTSD are themes in DS2. This is what Kojima meant when he said that he made changes to the game. This is a guy that denied people a bossfight with the antagonist just to ruin any feeling of catharsis. He could have easily written a wholesome journey for Sam which he deserves but Kojima always puts his protags through insane amount of suffering.
