It'll fall under the same general category of "There's a fine line between something horrific and something that sounds really stupid". At that point, it may be something the people that would inhabit that universe take as fact, but it would be something the readers of your content might debate heavily on whether it falls under grimdark or grimderp.
Basically, if there are people suffering the black plague or something in your setting, that's one thing. But if it gets to the point you could make a chinese history meme out of the sheer body count or make the disease sound something akin to a zombie plague, then people are going to start rolling their eyes.
