A lot of problems with the game stems from the fact that they did the two faction system.

Each racial faction should have been its own faction, and depending on which race you picked you either start off neutral, or unfriendly with the other factions, with old allies being friendly, like humans and dwarves, and old enemies being hated, like elves and trolls.

Then instead of having every single fucking conflict in the game be Orcs and Pals vs Humans and Friends, you can instead have different faction conflicts at different times with different contested zones and people can opt-in to fight for those factions if they're at least honored with that faction. When it comes to who players group up and guild with, who gives a fuck, an undead teaming up with a human to do scholomance who gives a fuck, players should be treated like mercs.

Better PVP would have also helped, but lets be real, replacing Arenas with CTF/KOTH/AFK Valley wouldn't have made the game better, they should have developed interesting guild vs guild systems in the open world, instanced honor farms were always lame and gay

And lets be real, people who fuck up their reputation and then whine they have a hard time progressing arent really something you should design your game systems around.
Regardless you could always have a way to get the minimum Neutral reputation for paying money or by doing faction quests that you could get from neutral zones.
