The foreskin is not cut off - not initially.
First it is crushed between metal plates and left in that state for five minutes to provide hemostasis. Only then it is amputated. Anesthesia does not exist or is insufficient because pain killers for newborns is dangerous. So sometimes they get a topical solution which doesn't do shit (all topical solutions are shit).
