I'd start by not making a sequel to Bloodlines and just make a standalone RPG not carried by nostalgia.
I'd take inspiration from Redemption though and separate the game between historical era and modern day.
As for the plot, anything works, it doesn't even need to go super crazy into vampire stuff, what's cool about the world of darkness is that you get to see so much fucked up shit and most of it doesn't have to come from a tzimisce turning people into chairs, there are billions of possibilities for quests.
Gameplay I'd go for turn based grid style.

It's just like Bloodlines 1 - seriously undercooked game with potentical to become beloved classic if dedicated fans spend another decade patching and modding it.

There's several versions (editions) of Vampire roleplaying system and each puts implicit emphasis on different aspects of the setting, but each can also be used (if perhaps less efficiently) for other themes.
