It depends on the studio, director and channel, the reason as to why the Ranma remake is so heavily censored for example is because it is animated by MAPPA and they have never ever done nipples (there are other studios who used to allow nipples like KyoAni and Doga Kobo but banned nipples from their shows once they got "big")
