It's pretty much the same. Few new pieces of gear, a little more freedom in creating a custom build, new mini-modes for the challenge (the best part), and that's it. Sometimes when you play through a game for the second time, even objectively increased difficulty settings like enemy attack and health or new enemy compositions often feel like an easier ride in the perceived experience than the first playthrough. The game has already been mastered, many of its secrets have been figured out, as a result of which the second playthrough does not become that different from the previous one in terms of the resulting experience and sensations. Even if you die more often in battles purely quantitatively, in the experience it seems as if there is no significant change. FF mode in XVI is an example of exactly this situation, and I replayed it with PC difficulty mods that added three times the damage to all mobs and bosses. Ultimaniac mode, on the other hand, changes the rules significantly, and is the best and most difficult mode that the game offers, however, it is limited to a list of story missions and in order to fully complete it, you must first complete the game literally 2 times, since each mission in Ultimaniac mode is only unlocked after its completion in FF mode. The gatekeeping level is simply inexplicable to me. One of the dumbest design decisions in XVI.

To put it briefly:
FF mode does not significantly improve anything, in my experience.
Ultimaniac is a mode worth checking out, but to fully unlock it you need to complete FF mode. Download some save, if you want to try it. Ultimaniac is a list of story missions where potions are not scattered around the map and the equipment is selected by the game itself, so you can't come overpowered. With PC difficulty mods (Modular Combat Rebalance), this mode is worth it.
