Carnevil scared the dick off me as a kid. Seeing it in arcades was a surefire way to make me have a nightmare. Its funny because even now just seeing the advertisements for it still makes me uncomfortable even though it doesn't necessarily "scare" me anymore.
mine was boss rooms in zelda games AFTER the boss is dead, my brother used to stand around just to shit me up because I had some looming anxiety that the boss would come back or something

My childhood fear is lightning. We were on a road trip and our vehicle got struck by lightning. Everything seemed fine but ten minutes later, one of our tires exploded while we were speeding on the interstate and we spun out into a median. No injuries, but scary as hell for a ten year old. I still get a little on edge in severe storms.

the paper mario series has a few of these. Like ghost t for example. Like he's harmless(usually) but the way the cheerful train music music cuts out when he spawns and the undead ghost theme played when tralking to him was unsettling as a kid. Reading his diary also triggers a special gameover.
