The anime lore explanation is that the famous horses of our world upon demise somehow send their spirits to this world that then enter into pregnant human women. They're then reborn as horse girls cursed to live out an identical fate to their rl world counter-parts (but more PG). The game is a little nicer since they can actually avoid some of that. Their personality generally will be influenced by the real world horse. You could argue it's all very fucked up but you aren't meant to think about it that deeply.
