Everything about DS2 feels different. Even the things that are the same on surface. Neil is almost an opposite of Cliff, Higgs feels like a proper main antagonist as opposed to his secondary role in DS1, the PTSD Louise changes the tone of the game massively compared to how you connect with it in DS1. Stuff like motorcycle race bossfight, pizza karate, coffin hoverboard, guitar weapon and other batshit crazy additions. You have a crew of weirdos instead of being alone. The soundtrack by Woodkid and main theme by Caroline feels so much more experimental.
It's just like how MGS2 used the same plot points from the first game but when you think about it they were nothing like MGS1 in the context.
