We don't have an official reasoning currently, all that we know based on circumstance is that it was a Toby Team thing (outside company like ESRB or Nintendo would've likely implemented emergency procedures if it was them + official statement, ESRB would've pulled shit from stores if it was deemed bad enough). Thus with no official answer there's 2.5 main theories.

That Toby, his entire team + playtester(s) didn't think about the physical rape interpretation at all during development and QA. Toby himself is a fucking weeb and I highly doubt he didn't catch this let alone everyone else.

The rose scene was something that was always contentious internally but won out for release until very early reactions forced their hand. Maybe it was just not being read as Toby + Team intended on ground zero reactions and changed to try and hit at what's at play more (and/or two clashing creative visions, the changes were done in the game where Gerson mentions doubt poisoning stories, I could easily see that as a writer venting from internal frustrations from Weird Route tip-toeing). AND/OR it could be Toby's team not wanting a [HOCHI MAMA] 2.0 controversy to come back but even worse, so Toby's team made a quick, slap-dash change to try and circumvent it (would explain the speed of the Steam patch as the other versions weren't altered while Steam got 2 changes to the scene). Furthermore the animation assistant was involved in backstage drama for Skullgirls that caused a creative, others are more knowledgeable than I, most I can gather is that it was a perv-scandal that also lead to self-censorship in the game.
Want to clarify, these are theories exclusively based on context because the only souls that have the real answers won't speak.
