Are their musculature different from humans? Are they an entirely different species from humans genetically speaking despite being practically human in appearance and function? Are there actual horses in the Umamusume universe? If there aren't any actual horses, why do they have the word horse? Where does the term "Umamusume" come from in-universe? Did these umamusume fulfill the roles of horses in-universe historically, such as pulling wagons and cargo or being ridden to battle? Once you answer one question, you'll have to answer the rest. So the better option is to just say "Don't worry about it" and enjoy the game and girls for what they are. There doesn't have to be a definite answer for everything.
